Privacy statement of Dragonfly Health Centre
Dragonfly Health Centre keeps a record of your medical and personal data in order to be able to carry out your treatment in the best possible way. To ensure that this data is handled carefully, certain rights and obligations are attached to it.
Naturally, this practice handles the data with care: therefore, in addition to the aforementioned Personal Data Protection Act, a number of rules have been laid down in a privacy statement that takes into account new legislation as of May 25, 2018.

Your personal and medical information will be kept confidential.
As a paramedic organization, the practice is forced by law and health insurance company requirements to store personal and medical data. This is done in a secure software system of our PC.
Only data related to treatment are stored in the medical file as a word document. The contents of these files are electronically protected by antivirus and spyware software and protected against loss through backups.
Your medical records will only be used for your treatment within the practice and possibly for consultation with other medical specialists or your personal trainer.
Except our team at Dragonfly Health Centre, no one else has access to your data and she has a duty of confidentiality.
You have the right to see the data recorded about you. If you believe that the data has been recorded incorrectly, you can request the centre to change it.
Paper referrals are carefully archived so that they cannot be inadvertently viewed by third parties.
Screens with personal data and/or electronic agendas will be protected with a pin code.
Medical data of clients is not released to third parties, including immediate family members. Data is released only after the client has given permission.
The legal retention period of your file is 5 years, this is in the interest of the client to be able to read back information and build a file. Clients may request that their file be deleted.
If you are of the opinion that our practice does not handle your data properly, you can make this known via the applicable complaints procedure within the practice or via the Personal Data Authority.

Treatment
There are also a set of guidelines for dealing with and treating clients that are used within the practice.
The procedure for treating a client will be explained during the treatment. This will indicate what actions will be performed. If there are any objections to the treatment, the client should indicate this immediately.
If there are religious objections to certain treatments and/or forms of treatment, the client should make this known at the first appointment so that the physiotherapist can assess whether the client can be treated.
If a client objects to being treated by a therapist of a different sex, this must be indicated before the first consultation.
The treatment generally takes place in a separate room where only the therapist and client are present. Only in group therapy will several people be present in the same room.
If third parties (e.g. a trainee) are present during treatment, the client's prior consent will be requested.
The therapist, and any supervisors, should behave respectfully toward the client and omit informality that is not desired.
Personal privacy is respected. Clients may, if they wish, undress and dress separately.
If situations arise that are not desired, in consultation with the client and therapist, another therapist will be used or the treatment will be terminated.
Discussed topics with the client remain confidential and will not be discussed with third parties, except when the client gives permission.
If you are of the opinion that the practice does not handle your data correctly, you can make this known via the applicable complaints procedure within the practice.
Complaints
We make every effort to prevent complaints. But if you are dissatisfied for any reason, please let us know. First, talk to the person(s) involved. There may be a misunderstanding and we will be happy to find a solution. If you cannot resolve it, you can file a complaint.
